Under section 503A, a compounded preparation is prepared for an identified individual patient on a valid prescription. Clear prescriptions make the review faster.
Include
- Active ingredient or ingredients, with strength or concentration.
- Dosage form and, where it matters, a preferred base or vehicle.
- Quantity, directions for use and refills.
- The patient-specific reason a commercially available product is not suitable, where applicable.
- Any excipients to exclude, such as dyes, lactose or alcohol.
Controlled substances
Prescriptions for controlled substances, compounded or not, must meet DEA and Pennsylvania requirements. Please send them by certified electronic prescribing (EPCS), or call the pharmacy first to confirm another legally permitted way to send them. Do not use the prescriber portal for them.
How to send it
Use the prescriber portal, e-prescribe to Cambria, or fax the printable order form. The Send a Prescription page lists each option and the details your e-prescribing system needs to find us.
Consult first if unsure
A pharmacist can talk through formulation options, stability limits and practical alternatives before you write. Call the pharmacy to arrange a consult.
